Purpose: To improve care for all Long-Term Care residents in Bucks County.
Mission: To provide leadership for coordination and standardization of information and resources for the Bucks County Long Term Care community.
BCHIP in conjunction with the Bucks County Commissioners, Bucks County Health Department, Bucks County Emergency Services, all Bucks County hospitals, and our long-term care facility partners created the Bucks County Long Term Care Partnership during COVID.
The partnership is a collaborative effort to create an open, non-competitive place to have discussions between facilities to identify shared resources and collaborative opportunities. . COVID showed all of us how important it is to know each other before, during and after these events by allowing our hospital members and community partners to share their strengths and lessons learned. We expect this work to provide valuable information and resources that will help the organizations and most importantly improve care for their patients.
We are hoping that all Bucks County long-term care agencies join the Long-Term Care Partnership, this includes nursing homes, personal care facilities, rehabilitation centers, respite and hospice agencies, and any other agencies that provide long term care. As we grow, we will expand with adding sub-committees. Our goal is to get input from all, share with all and hopefully be able to assist all.
